Goals of School Health Services Program

  • Identify health problems and needs of students and staff.
  • Promote optimal level of health for students and staff.
  • Assist students in becoming increasingly responsible for their own health.
  • Promote environment5al safety and health within the school and during school related activities.
  • Provide health education and health counseling for students, parents, and staff.
  • Provide direct health services in an efficient and cost-effective manner
  • Maintain a liaison with primary health care providers

Health Conditions

It is my goal to identify and safely care for students with acute and chronic health conditions.  The parent/legal guardian is responsible for completing the Annual Health History Form at the beginning of each academic school year and contacting the school nurse if individualized health plan or emergency action plan, medications (daily or "as needed", skilled procedures, adaptive devices, etc. are needed at school.

Parents/legal guardians are responsible for notifying the school and school nurse if and when emergency contact information changes.

Student Transfers

Students transferring from another school system, another state, or another country must present the most recent certificate of immunization. North Carolina immunization and school attendance laws require that parents/guardians provide an up-to-date immunization record as proof that their child received all required immunizations. For students attending public schools in North Carolina for the first (1st) time, a health appraisal (physical) is also required. The NC Health Assessment Form must be completed by a NC licensed health care provider. All required documents must be received within 30 calendar days from the first day of school attendance.  If the required documents are not received by the start of the 30th school day, your child will be excluded from school and will be allowed to return when the documents are received.
